    Our social work services at Rainbow Centre aims to improve the quality of life of persons with disabilities through the delivery of quality services that support and empower these individuals, their family members and the communities around them so that they can achieve their highest level of independence and lead thriving lives.

    Bringing to each case an understanding of the biopsychosocial concerns of families and the influences of family, community and cultural differences, the Social Worker is responsible for providing high quality and effective case management. The Social Worker acts as a liaison between the client, healthcare providers, and community resources to coordinate comprehensive care and support services and advocates on behalf of clients to ensure they receive the necessary assistance to live as independently as possible.

    Key Responsibilities

    Client Needs Assessment and Care Planning

    • Assess clients' physical, mental, emotional, and social needs through observation and interviews, even in complex cases
    • Set goals using standardised and non-standardised assessments
    • Plan, implement, and adjust care plans based on clients' goals
    • Work with other professionals to create integrated care plans

    Care Coordination and Service Delivery

    • Provide individual and group interventions as part of a transdisciplinary team
    • Coordinate services and track client progress
    • Manage caseload based on priorities, waiting lists, and outcomes
    • Develop community programmes suited to client needs
    • Provide crisis intervention and short-term counselling when needed
    • Complete case notes and reports as required

    Advocacy and Systems Navigation

    • Advocate for clients within healthcare and social service systems
    • Help clients access services and overcome barriers
    • Join multidisciplinary meetings to review and adjust care plans

    Continuing Professional Development

    • Reflect on and evaluate own practice regularly
    • Stay updated and compliant with relevant laws, policies, and professional codes

    Key Requirements

    • Recognised degree in Social Work
    • Registered as an accredited Social Worker with SASW
    • Experience working in a similar setting will be an advantage
