The Adult Empowerment Services deliver person-centred services that support adults with disabilities to develop skills, confidence, independence, and valued social roles across daily living, community participation, and meaningful engagement. Services span centre-based, community-based, and case management contexts, and are guided by each client’s Good Life Care Plan (GLCP).
The Life Coach works proactively with clients to translate person-centred plans into meaningful daily experiences, coaching individuals to build capabilities, make choices, and participate more fully in their communities. Through consistent practice and collaboration, the Life Coach contributes to positive client outcomes, service quality, and a safe, supportive environment for clients and families. |

Care Facilitator [Day Activity Centre]

Through services delivered across the life course, RC supports individuals to develop skills, independence, and meaningful participation in everyday life—from out-of-school-hours care and education to adult empowerment, therapy, social care, and community integration.
The Care Facilitator is a key support role that assists in the delivery of daily activities and routines for persons with disabilities. Working as part of a team, the Care Facilitator provides hands-on support during activities, transitions, and personal care where required, helping to ensure that activities are safe, engaging, and well-organised. Guided by each individual’s Good Life Care Plan, the Care Facilitator provides consistent, individualised support that helps create structured and nurturing environments where clients can participate meaningfully, practise daily living and social skills, and experience positive interactions with peers and staff. |

The Senior Executive, Shared Services will be stationed onsite in the School Administration Office for 4 days a week and he/she will spend the remaining 1 day in HQ office to stay aligned with organisational HR policies and practices. |
